Medical Misprints/Bloopers (original)

In my position, I come across many bloopers that transcriptionists type:
1. The patient had a wide-based gate. (obviously very painful)
2. The patient wears a 38D brazier (brazier is a person who works with brass, so patient must have had brass boobs)
3. The patient had a repeat lumperoscopy.
4. The patient was referred to Dr. Smith with food sticking to her.
5. The consolation of symptoms suggests a diagnosis of...
6. The patient's urine forecasts was sent.
7. The patient takes Tylenol and lyes down.
8. The patient was examined on the table standing.
